WHAT IS A PERSONAL HEALTH RECORD (PHR) OR PERSONAL MEDICAL RECORD (PMR)?

A personal health record or personal medical record is not the same as an electronic medical record (EMR).

A PHR/PMR is an individual's medical record that contains medical information which the patient has stored and maintained on their own. Personal medical health records are maintained in many forms including handwritten, typed, or some other format including electronic. They are NOT controlled by or maintained by an outside institution such as an insurance company or institution. Some patients may choose to share their personal information with others but this is under the control of the patient.

An EMR (electronic medical record) or EHR (electronic health record) is owned, controlled, and maintained by an institution such as a doctor's office, hospital, or health insurance plan.

A PHR/PMR that is tied to an EHR is called a patient portal.
